Human Services Professional

Our client is immediately seeking a full-time Human Services Professional to join their team at their Kapolei office. As a Human Services Professional you will evaluate patient's needs and develop and monitor treatment plans.

Responsibilities

• Identify and evaluate patient's needs.
• Develop service and treatment plans.
• Monitor services being provided.
• Assess patient's progress.
• Make changes to treatment plans as needed.
• Perform other duties as assigned.

Qualifications

• Bachelor's degree with a minimum of twelve (12) semester credit hours in the following courses: counseling, criminal justice, human services, psychology, social work, social welfare or sociology.
• Two years of professional work experience helping individuals and their families identify their problems, cope with their conditions and function effectively within their environment.
• Valid driver's license preferred.
• Strong communication (verbal, written, interpersonal) skills.
• Strong analytical, organizational, multitasking and problem-solving skills.
