Job summary
We are seeking a reliable and compassionate individual to join our team as a Housing Navigator who will provide comprehensive support to individuals facing housing instability. This position will act as an advocate, provide case management, assist with benefit establishment, connect our participants with mental health and substance abuse services, and link them to other necessary supportive services. Additionally, the Housing Navigator will develop individualized plans to address barriers to housing and ultimately help participants maintain and secure permanent housing.

Summary of Essential Job Functions
- Perform initial comprehensive case management assessment and develop a plan of intervention that addresses need for housing, services, crisis intervention, and continuity of care planning and coordination.
- Provide services designed to help participants and their families obtain stability.
- Provide onsite case management services in the areas of social events, employment, benefits establishment, and other services as needed to assist in reaching housing plan.
- Identify each area in which participants might need assistance to accomplish their outlined goals and objectives (i.e. Scheduling appointments, applying for public benefits, identifying subsidized housing, etc.).
- Encourage and promote an environment that is strength based to assist in meeting housing goals.
- Maintain participant related data tracking systems, including case notes and complete HMIS entries.
- Prepare case-related reports and generate data for monthly reporting.
- Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with state, county and the organization’s guidelines.
- Identify new and existing opportunities and build strong relationships to better assist participants in accessing resources, employment, supportive services, and housing opportunities.
- Attend collaborative meetings and actively participate in staff meetings and training.
- Network with other agencies, coalitions, and local community meetings.
- Other duties as assigned.
Minimum Educational Requirements
- A bachelor’s degree in behavioral sciences is preferred or a combination of education and related experience equal to four (4) years.
- Minimum requirement of two years related experience in case management services, housing, homeless and mentally ill populations and knowledge of Public Housing Authority and Housings subsidy (i.e.. Section8, Shelter Plus Care, VASH).
Abilities Required
- Computer skills with proficiency in Microsoft Office. HMIS training a plus.

- Highly motivated self-starter and the ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment.
